What You'll Do
- Evaluate patient admissions and determine appropriate levels of care using established clinical criteria
- Develop, implement, and oversee individualized discharge and transition plans
- Assess patients' clinical, psychosocial, environmental, financial, and post-acute care needs
- Coordinate services and community resources to ensure smooth transitions across the continuum of care
- Collaborate with physicians and interdisciplinary teams to optimize patient outcomes
- Facilitate care conferences and interdisciplinary meetings
- Partner with insurance providers and third-party payors to secure authorizations and minimize barriers to care
- Monitor utilization and admission status to ensure compliance with established guidelines
- Advocate for patients and families while supporting safe, effective discharge planning
- Participate in quality improvement initiatives and contribute to enhanced patient satisfaction outcomes
- Support palliative care coordination and family-centered care discussions when appropriate
- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license in the state of practice
- Basic Life Support (BLS) certification upon hire
- Minimum of three years of clinical nursing experience in an acute care hospital
- Care Management experience required
- Skilled nursing facilities
- Outpatient case management
- Home health
- Hospice
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Master of Science in Nursing (MSN)
- Utilization Management or Quality Management experience
- Specialty certification
- Knowledge of third-party reimbursement processes
