PeaceHealth is seeking a Clinical Supervisor Home Care Services - Hospice Medicare Certified for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $55.98 – $83.96. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
Job Summary
This position is responsible, under the direction of the Manager, for providing direct supervision and clinical oversight of the clinical multidisciplinary team which may include management of direct patient care staff and overall coordination of the team's clinical services as evidenced through outcomes.
Essential Functions
- Oversee and coordinate day-to-day operations of the assigned clinical services team, including provision of accurate clinical documentation specific to the care plan. Ensures quality patient care/outcomes along with complies with industry and agency standards.
- Manage, supervise, coach and train direct patient care staff while providing leadership and serving as a resource in the overall delivery of home health/hospice services. This includes participating in hiring, firing, and staff performance/competency evaluations.
- Coordinate team’s activities to ensure efficiency in the delivery of patient care services and systems/operations, cost-effectiveness, staff productivity and timely/accurate communications throughout the continuum of care.
- Ensure that services are provided in compliance with regulatory requirements and department policies and procedures.
- Coordinate clinical/patient care services cost-effectively within established budget. This includes productivity and benchmarking outcomes within industry standards. Participate in budget planning and monitoring.
- Perform program evaluations and prepare written reports as necessary, utilizing data collection and analysis concepts.
- Facilitate interdisciplinary team and case conference/meetings and regular one-to-one case lead reviews with case managers and other members of the clinical staff.
- Manage/coordinate services for Home Health, Hospice, and/or Palliative Care patients.
- Demonstrate knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs
- Performs other duties as assigned.
Qualifications
Education
- Accredited Program Required: Nursing Minimum of an Associate's Degree or
- Accredited School Required: Physical Therapy or
- Accredited School Required: Occupational Therapy or
- Accredited School Required: Speech and Language Pathology or
- Accredited School Required: Social Work
Experience
- Minimum of 2 years Required: physical therapy, occupational therapy, speech language pathology, social work or registered nurse in home health, hospice, or palliative care within the last 5 years.
- Preferred: Previous leadership experience
- Preferred: Knowledge of regulatory guidelines specific to CMS and state agencies
Credentials
- Required: Current State Licensure as PT, OT, SLP, RN, Social Worker Advanced License, Social Worker Associate Advanced License, Social Worker Associate Independent Clinical License, or Licensed Independent Clinical Social Worker in state of practice and
- Required: Upon Hire Basic Life Support and
- Required: Driver's License and proof of auto insurance
Skills
- Strong organizational, administrative and problem-solving skills. (Required)
- Excellent communication skills, both verbal and written. (Required)
- Proficient computer skills including MS Office applications as well as clinical applications. (Required)
Working Conditions
Lifting
- Consistently operates computer and other office equipment.
- Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
- Sedentary work.
Environmental Conditions
- Predominantly operates in an office environment.
- Some time spent on site in medical/hospital setting.
Mental/Visual
- Ability to communicate and exchange accurate information.
- The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
