The Community Health Coordinator is responsible for the planning, funding, and management of public health programs and services for children and adults in Skagit County. Under the framework of Foundational Public Health Services, incumbent builds capacity within the local community through active partnerships with a variety of community partners to improve health outcomes for strategically identified populations.
- Salary upon hire is in accordance with Skagit County promotion policy.
- Progressive increases are based on years of service with Skagit County, up to 10 total steps (top step $9,260.51/mo.). The Community Health Program Coordinator is range 28 of the General (non-union) salary schedule.
Essential Functions

Maintains daily responsibility for the administration and management of community health programs through assessment, policy development and assurance, under the direction of the Division Manager.
Plans, organizes, and determines overall program mission, goals, objectives, core services and allocation of financial resources in alignment with strategic priorities of Public Health, Skagit County and the Foundational Public Health Services framework. Seeks opportunities for collective impact by coordinating work with the efforts of the Population Health Trust.
Develops, implements, and/or contracts for programs and services that are evidence based, fiscally responsible, and culturally relevant for Skagit County residents.
Fosters and supports role of assurer through outreach, education, facilitation, consultation and community planning. Convenes and facilitates planning efforts across systems to work in partnership to set shared goals and strategies to improve the health of the community. Provides training opportunities for community groups and providers to meet the changing needs of consumers and families.
Embeds Foundational Public Health Services framework in decision making, planning, and outreach to effect system change in Skagit County. Provides ongoing education and outreach to staff, community organizations and providers to strengthen understanding and alignment for creating pathways for health improvement. Provides, linkage and system coordination to ensure full utilization of and access to publicly funded services.
Utilizes a data driven approach to planning, program development, and decision-making using evidence based strategic planning methods as a method for identifying a process for achieving outcomes. Conducts on-going program evaluation and makes recommendations for improvement to accomplish goals.
Negotiates contracts with County sub-contractors and provides management, oversight and program monitoring for projects funded through Skagit County. Conduct Requests for Proposal (RFP) processes as needed, and develop biennial program plans and budgets. Conducts on-site monitoring for contract compliance both legally and programmatically and for achievement of outcomes. Establishes continual technical assistance and in person training to our contracted providers and/or terms for corrective actions and enforces sanctions as necessary. Works cooperatively with community providers to improve and problem solve programming issues.
Supervises program staff, outlines work priorities and assignment, provides direction and coaching for delivery of services, and assures quality in compliance with funding requirements and department goals. Recruits, trains, directs, and evaluates program staff.
Prepares grant applications and reports for funding. Develops and monitors budgets and program expenditures as needed.
Represents Skagit County Public Health in community, interagency and State meetings and partnerships.
Responds to community concerns and complaints; assures program activities are in compliance with laws, policies, regulations, and goals. Identifies and addresses customer needs. Emphasizes public accountability and a positive public service approach with assigned staff.

Directly Supervises staff.

Minimum Qualifications
-
Any combination of training and experience that would provide the required knowledge, skills and abilities is qualifying. A typical path to obtain the required qualifications would be:
- Bachelor’s degree in health or social sciences; or closely related field. Master’s Degree preferred.
- A minimum of three to five years’ professional experience in program design, development, and oversight.
- Experience working with health and community service providers and community groups.
COVID-19 vaccination is a required condition of employment for Skagit County Public Health. Proof of vaccination is required upon hire.
REQUIRES KNOWLEDGE OF:
- Principles and practices of population-based health and Foundational Public Health Services/Capabilities.
- The field of assignment sufficient to perform completely and accurately the full scope of responsibility as outlined in the position description.
- Available public and community health resources and community groups within Skagit County, as well as regionally and Statewide.
- Grant writing and program development.
- Planning and facilitation principles.

LICENSES and/or CERTIFICATIONS:
Valid Washington State driver’s license
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
Work is performed primarily in an indoor office environment and requires the ability to work at a desk and computer for long periods. Sufficient hand-eye coordination to operate office equipment. Attendance at meetings and some travel involved.
HAZARDS:
Those present in a health care and office environment. Relationships dealing with the public.
