Basic Function
Provides oversight and assures performance for Incident Investigation, Learning from Incidents, and Continuous Improvement processes. Responsible for conducting or facilitating the investigating and determining the root causes of accidents, incidents, and near misses at the facility. Works closely with management and staff to identify underlying issues and develop solutions to prevent future occurrences. Act as a change-agent to implement new programs to meet corporate requirements and improve existing systems to increase the overall effectiveness of the program.
Job Duties
(functions considered essential as defined by ADA)
- Leads local implementation and assurance of the OEMS Investigation and Learning Standard
- Conducts thorough investigations of accidents, incidents, and near misses to determine the root cause(s)
- Gathers and analyzes data to support the investigation and identify contributing factors
- Develops and tracks the implementation of corrective actions to prevent similar incidents from occurring in the future
- Provides training and coaching to management and staff on incident investigation and prevention techniques and learning sessions
- Updates and enters Incidents, Investigation Details and Corrective Actions into Enablon, or provide assurance for entry of same, for tracking to completion
- Facilitates the CL/LFI Steering Team (select investigation, process improvements, KPI review)
- Closes coordination with the USW TOP Representative
- Maintains the CL Skill pool through initial and refresher CL training
- Coaches teams and sponsors for each investigation
- Develops a Level 2 investigation team rotation schedule for forthcoming year
- Participates in CL network telecoms and face-to-face meetings
- Leads with the Manager, HFS Corporate Investigations and Learning, and HFS Legal to ensure the integrity, quality and confidentiality of LFI assessments incident investigations, as appropriate
- Ensures that LFI reports are shared with Corporate Learning Co-coordinators appropriate disciplines in a timely fashion
- Reviews learnings from other sites for applicability
- Maintains accurate records and documentation related to investigations
- Maintains the TOP/Causal Investigation SharePoint site
- Analyzes Power Bi Metrics that identify trends, gaps and opportunities for improvement.
- Recommends and facilitates Enhanced Problem-Solving Teams(s) as warranted by multiple assets with significant, linked performance problems
- Coordinates with the PSR Assurance Focal Point to facilitate periodic Assurance activities across CL activities
Experience
A minimum of 5 years of job-related experience in a refining operational or maintenance capacity or technical support discipline.
Preferred Experience:
10 years of related experience is preffered.
Education Level
A minimum of a Bachelors Degree in a technical field (e.g. Science, Engineering, etc.) or related work experience in lieu of degree.
Required Skills
A personal passion for problem solving and continuous improvement. Formal and informal leadership skills; ability to influence people outside of direct reporting structure. Knowledge and experience in the Causal Investigation and Learning from Incidents processes. Meeting and Team facilitation. Auditing/Assurance skills and experience. Knowledge of refinery operations, maintenance, and work processes engineering practices. Ability to effectively communicate with others through both written and verbal communications. Goal oriented and organized. Advanced reading and technical writing skills, with the ability to perform intermediate mathematical calculations. Working knowledge of Microsoft Word, PowerPoint and Excel. PRIMARY INTERFACES:

Work Conditions
Office based with up to 20% travel by land, air and water required. Petroleum refinery, warehouse/plant environment, out-of-doors, driver-based environment including but not limited to chemicals, pressure vessels, tanks, rotating equipment, working in confined spaces, and exposure to the risk of electrical shock. Subject to all temperatures, weather and varying road conditions. Ability to work extended or off-shift hours as required.

Physical Requirements
Job conditions require standing, walking, sitting, twisting, stooping, crouching, kneeling, talking or hearing, making visual inspections, making precise hand and finger movements, reaching or grasping, climbing up to 25 ft, required to wear personal protective equipment as needed and a respirator in emergency situations (beards not permitted), lifting and/or carrying up to 15lbs, ability to operate and drive all assigned company vehicles at company standard insurance rates is essential, valid state drivers license and proof of insurance required, and perceiving color differences.

Salary Range: The salary range includes the minimum, midpoint, and maximum base salary values. A candidate's offer can vary depending on differentiating factors, including but not limited to education, training, or experience.
This position has a salary minimum of $115,300.00 and maximum of $200,700.00.
